I think that medical insurance will be tracked by 2 sources.
source 1. All of those who file taxes. It has all the dependents on it.
source 2. those on social security or on public assistance. That would track all so-called illegals and those who have low or no income that dont file taxes.
I frankly cannot think of who would not have a source of income and live in US except for visitors and real illegal immigrants.

You know, it's like saying 'I don't have children so why should I pay property taxes, part of which covers public education in my district?"
As for who would enforce medical insurance - the beauty of healthcare in Canada is that you're automatically covered if you're a Canadian citizen. There's no need to enforce insurance.
