The rebate form is eligible for a rebate if your plan is the Sidekick Voice-Barred/Hearing Impaired rate plan of 29.98/mo. I believe you are on unlimited data plan which is $29.99/month. So therefore you didn't qualify for that rebate, you are pretty much ruled out. I'm sorry that happened to you.
