Good question. My guess is they dealt with her substance abuse first. Then, set about getting an accurate diagnosis. I think it would be difficult to get an accurate diagnosis if the person was always on mind altering substances?

Exactly. Can't get an accurate dx when a patient is using mind and mood altering drugs. Likewise, depending upon what they are using, giving an antidpressant or a mood stabilizer while they are abusing could lead to some serious consequences...including death.

of course. i guess i was thinking about the immediacy of the manic episode and bringing spears down from it, but you're absolutely right. i failed to consider the fact that mood stabilizers and antidepressants mixed with alcohol and drugs could create a life theatening situation or death.
