Deaf terminology??

I'm signing up for membership with my provinces deaf and hard of hearing service thing and on the forms it asks you to identify as

Deaf
Late deafened
Hard of hearing
Hearing

I have no idea what they mean, help lol?
 
Deaf= born deaf or culturally Deaf
Late deaf= lost hearing when older but born hearing
HOH= nor hearing not deaf, normally wear HA
Hearing= hearing...
