Depends on what type of work meaning full time/part time or contract type work.
Based on full time or part time- best bet is to talk to your supervisor or manager they then can talk to HR and go from there (if there is some sort of department for EEO I think it is- can't hurt to ask them).
If the manager isn't doing anything or won't, contact Human Resources directly and they can probably help or guide you.
If you are a contractor- you need to talk to the contracting company you work for- they'll get the ball rolling and either provide it themselves or speak with the correct department in the client site.
