Responsibilities:
- To efficiently provide client-centric and wrap-around psychosocial needs of deaf adults with chronic and intensive mental health diagnoses and their family/guardians, that are linguistically and culturally responsive to all, while ensuring Medicare Joint Commission, and hospital standards are being met.
- Perform trauma-informed psychosocial assessments/social histories, post-hospitalization transitional planning, and therapy and casework with deaf adults who are severely and chronically struggle with mental health symptoms and diagnoses, in American Sign Language or other visual communication mode required by the communication assessment.
- To provide essential social work services to include treatment team participation, psychosocial assessments, documentation per protocols, placement/discharge planning, and regular family contacts.
