The New York Foundling is currently hiring for a Case Planner for our Deaf Services department. The Case Planner is responsible for providing Case Management services for 12 families in which their is a Deaf family member throughout the 5 boroughs.

The New York Foundling is currently hiring for a Case Planner for our Deaf Services department. The Case Planner is responsible for providing Case Management services for 12 families in which their is a Deaf family member throughout the 5 boroughs.
Responsibilities
- Provide case management services to 12 families in which there is a deaf family member.
- Document family assessment service plans (FASPs), progress notes in Connections and Evolv system, enter contacts into PROMIS in a timely manner.
- Conduct comprehensive family assessments, quarterly safety reviews, Domestic Violence screening, CRAFT and CAGE screening.
- Develop services plans with the family that identify problems that are effecting their daily functioning in the home, school, community and extended family.
- Maintain monthly contacts with families per ACS requirements.
- Meet with ACS, school staff and other community providers as needed, regarding the service needs of the famliy and coordinating shared services.
- Provide information about other community services as well as refer families for services.
- Attend all monthly staff meetings, weekly supervisions and assigned trainings.
Qualifications & Experience
- BA/BS in Social Work, Psychology, Sociology, or Counseling
- Fluent in ASL
- Bilingual Spanish speaking preferred
- Will require fieldwork throughout all boroughs of New York City.
