Hi Everyone,

The New York Foundling is currently hiring for a Case Planner for our Deaf Services department. The Case Planner is responsible for providing Case Management services for 12 families in which their is a Deaf family member throughout the 5 boroughs.

Responsibilities
  • Provide case management services to 12 families in which there is a deaf family member.
  • Document family assessment service plans (FASPs), progress notes in Connections and Evolv system, enter contacts into PROMIS in a timely manner.
  • Conduct comprehensive family assessments, quarterly safety reviews, Domestic Violence screening, CRAFT and CAGE screening.
  • Develop services plans with the family that identify problems that are effecting their daily functioning in the home, school, community and extended family.
  • Maintain monthly contacts with families per ACS requirements.
  • Meet with ACS, school staff and other community providers as needed, regarding the service needs of the famliy and coordinating shared services.
  • Provide information about other community services as well as refer families for services.
  • Attend all monthly staff meetings, weekly supervisions and assigned trainings.

Qualifications & Experience
  • BA/BS in Social Work, Psychology, Sociology, or Counseling
  • Fluent in ASL
  • Bilingual Spanish speaking preferred
  • Will require fieldwork throughout all boroughs of New York City.
