Hello everyone. I'm in the process of gathering information for a dissertation paper that discusses deaf access to emergency services (911). I need about 100 responses to my survey at:

I completed the survey but this is what I got??? Maybe you need to fix something there...

This is what I got when I submitted the survey.

Sorry about that. I forgot to put in the html code for my email. *blushes*

No need to click the link after you do the survey - just close the page (my intent is to gather information, not link to my page - I'll try to fix that).

Please let us know when the website is fixed the correct way to submit our surveys to you :) :thumb:
 
All fixed now. The form has been edited, tested and should work. Thank you all for your patience!
 
I wanted to thank those of you who have taken the time to do the survey. For those of you who haven't , I would really like to hear from you. I still need 80 people to take the survey!
