I know this may sound like a novel concept, but some people, (even porn stars) get off on pain. Masochism is nothing new and certainly didn't pop into existence with the start of pornography.
Don't be quick to make the assumption that something that physically hurts is painful or unwanted on the part of the person being hurt.
